CEHCHV holds a long standing partnership with the American Lung Association (ALA). Through partnership with the ALA, we provide asthma training programs as part of the Asthma Educator Institute, consultation to the Asthma Management in Schools Program and Tobacco Cessation Program. Through our esteemed colleagues at the ALA, we participate in activities with the Hudson Valley Asthma Coalitition (HVAC). Together we advocate for the best care for children with asthma or tobacco cessation resources. In the past we have worked together to promote home visiting nursing services for children with asthma throughout the Westchester and Hudson Valley region. Current work includes training healthcare providers on asthma management and working with local Head Start Programs in Dutchess County to provide early identification and screening for children with asthma.
CEHCHV is proud to partner with Feeding Westchester to provide comprehensive services to our patients who are experiencing food insecurity. During our clinical office visits, all of our patients seen are screened for food insecurity using the validated Hunger Vital Signs two question screening tool. Any patients who screen positive are referred to our Food Insecurity Team for comprehensive services and evaluation with our Social Worker, Nutritionist and referral to our community partner, Feeding Westchester. Patients can also self-refer to this service and any provider can submit a referral for this comprehensive program.
CHILDREN’S HEALTH AND RESEARCH FOUNDATION
The CEHCHV team works closely with the Children’s Health & Research Foundation to ensure that their synergistic missions of improving health services for mothers and children in the Hudson Valley region.
